1.     Fill the template form below correctly,

2.     The change cost N3000 with the card. Excluding the card is N500

3.     Send the filled template as an attachment to palmerisrael22@gmail.com

4.     The slip containing the change will be sent back to your mail in a PDF format, so you can download and Print in your Café. This process takes 10 minutes after your payment has been acknowledged.

5.     Fill each student’s details independently.
